What is MyNearNorth Patient Portal?

MyNearNorth  Patient Portal is a secure on line website that gives patients convenient 24-hour access to personal health information from anywhere with an Internet connection. Using a secure user name and password, patients can view health information such as:

  • Request appointments
  • Check your lab results
  • Leave a message for your Care Team
  • Request my medical records

How does the Patient Portal work?

Y ou can access MyNearNorth Patient Portal from any PC or mobile device with an Internet connection.Click on the MyNearNorth  Patient Portal link on the Near North Health Center website www.nearnorthhealth.org OR go directly to nn.emrconnect.org .From this screen, you can create your account and log-on to request appointment, check lab results or access pieces of your medical record. If you cannot locate specific information, please contact the  Electronic Medical Record Coordinator at 312-337-1073 x4336 , Monday through Friday from (8:30 a.m.- 4:00 p.m.) CST.

How do I register for MyNearNorth Patient Portal?

1. Go to the patient portal website

2..Click on "Register" on the upper left side of your screen.

3. Click on Register with Pin.

4.  Read the Registration Consent Form.

5. Click on the "I Accept" button if you agree to the terms.

6. Enter the following Account Information:

  • First Name
  • Last Name
  • e-mail address
  • Re-enter e-mail address

7. Enter the following Log in Information:

  • User ID ( Your Pin Number)
  • Type in New Password
  • Re-type New Password

8. Enter the following in Password Recovery:

  • Select 1st question from drop down box
  • Type in the answer to 1st question
  • Select 2nd question from the drop down box
  • Type in the answer to 2nd question

9. . Click on "Save" button.

How do I reset my password, or change my temporary password?

To reset your password, go to MyNearNorth Patient Portal login screen.  Click on the "I forgot my password" link. 

You will be prompted to enter your User ID or Email address. Then click "Reset Password". 

Is my information secure in MyNearNorth Patient Portal?

Yes. MyNearNorth Patient Portal is a secure Internet site which is protected through advanced encryption technology. Access to your account is secured through your own personalized user name and password, known only to you. It is important that you keep this information in a secure place and do not share it with others.

How do I view  my messages?

Go to the “Message Center” .

  •  In Secure Message box on upper left side of page click "View my in box”.
  • To view emails that you sent click on each message.

How do I leave a message to my Care Team?

- To viewsend us an e-mail, click “View my in-box” and click on "Compose".

- Click on “To” to find and select recipient, then click " Ok".

- Type in your "Subject" and in the space below enter your message.

- Click on "Send".


How do I log out?

Click the "Log out" link at the top right side of the page. The "Login" screen will appear verifying that you logged out successfully. You may then exit from MyNearNorth Patient Portal page.


Can I Request an Appointment using MyNearNorth Patient Portal?

Yes. From the portal home page:

1. Click on Request an Appointment

2. Click on New Appointment

3.Complete all required information

4. Click on Submit

 Can I Request a Medication Refill using the Patient Portal?

The preferred and quickest method to refill your medications is by contacting your pharmacy directly.


How do I Request my Medical Records?
1. Click on Request My Medical Records

2. Complete the following required sections:

  • Select your Near North Health Site from the drop box
  • Select Your Provider Name from the drop box
  • Click on the Medical Records you are requesting
  • Type in details the date of the medical records you are requesting and as much details as you can provide about the request. * Include the provider name, address, office phone number and fax number if you are requesting your records for a provider not with Near North Health.
  • Select the delivery method of the records ie mail, fax, pick up in person or electronic
  • Select and print the Authorize to Release Medical Record Form
  • Complete the form
  • Fax or bring the completed form and a copy of your picture ID to Health Information Department
  •  Click "Submit"

What should I do if I see a discrepancy or information that is not up-to-date on my chart?

If you see a discrepancy or have an update to provide our clinic, you can send a message to Request My Medical Records  using the Patient Portal, or you can call Health Information Management office at  (312) 337-1073.

Who do I contact if I'm having difficulties logging into or locating information in MyNearNorth Patient Portal?

Please contact the  Electronic Medical Records( EMR) Coordinator at (312-337-1073), Monday through Friday from 8:30 a.m.- 4:00 p.m. CST (excluding federal holidays)

While we’ve all accepted e-mail as easy and convenient, there are some concerns to consider.  

Although our system uses encryption technology for secure transmission, all Internet communications are at risk for interception by hackers, and anyone who has access to your e-mail can view the contents.  

E-mail addresses provided by employers are subject to their rules of ownership of e-mail transmission and content. Do not share or leave your user name or password with anyone. Always log out when you are finished with your session .

Appropriate Use

E-mail is convenient and appropriate for general questions, both medical and non-medical of a non-urgent nature.
